<p>always keeping my eyes open for the latest developments and when I recently changed primary physicians I purposely looked for one that utilized <a class="zem_slink" title="Electronic health record" href="http://en.wikipedia.org/wiki/Electronic_health_record" rel="wikipedia">electronic health records</a>.</p>
<p>It appears that some of the first ingestible smart pills are about to be commercially available. You can learn more in Brian Dolan&#8217;s article for <a href="http://mobihealthnews.com/15820/lloyds-pharmacies-to-sell-proteus-smart-pills-sensors/">mobihealthnews</a>. I&#8217;m looking forward to someday taking an ingestible smart pill that will wirelessly communicate with my computer to track and analyze my health, such as:</p>
<ul>
<li>Did I remember to take my medications today and at what time?</li>
<li>What did I eat today? How many calories were involved?</li>
<li>How much exercise did I get? At what times and in what intensity?</li>
<li>What vitamins, minerals, or other <a class="zem_slink" title="Trace element" href="http://en.wikipedia.org/wiki/Trace_element" rel="wikipedia">trace elements</a> do I need to consume more of?</li>
<li>Has there been an increase or decrease in x or y that indicates a concerning health issue I should be aware of?</li>
<li>How much stress did I suffer throughout the day?</li>
<li>How much sleep did I get and what was the quality of my sleep?</li>
</ul>
<p>I think such personalized medicine could help bring our healthcare costs under control. Think about it&#8230;</p>
<ul>
<li>We&#8217;d be reminded to take important medications.</li>
<li>We&#8217;d easily hold ourselves accountable for our caloric intake and exercise.</li>
<li>We&#8217;d increase the nutritional value of our intake as we saw areas in which our diet was specifically weak and were able to track progress in remedying this situation over time.</li>
<li>We&#8217;d be able to determine exactly what supplements would be helpful for us.</li>
<li>Numerous health issues could be detected long before they became a crisis &#8211; avoiding numerous hospitalizations and other emergency procedures which are extremely expensive.</li>
<li>Diabetics and others with chronic illnesses would be able to constantly monitor their condition.</li>
</ul>

			<content:encoded><![CDATA[<p>I love when technology solves everyday problems &#8211; and there is a bright future looming for the medical industry due to innovations in the technology field. <a href="http://radar.oreilly.com/2011/09/big-health-advances-in-small-p.html">Andy Oram has a good post highlighting some of the impending technologies here</a>.</p>
<p>I don&#8217;t want to take a position on nationalized healthcare b/c I&#8217;m sure to offend someone and I don&#8217;t feel qualified to comment authoritatively on the subject, but I do humbly suggest that one of the best ways we can bring healthcare costs under control is by <a class="zem_slink" title="Innovation" href="http://en.wikipedia.org/wiki/Innovation" rel="wikipedia">technological innovation</a>.</p>
<p>Andy writes in his post,</p>
<p>&#8220;<em><a class="zem_slink" title="Telemedicine" href="http://en.wikipedia.org/wiki/Telemedicine" rel="wikipedia">Telemedicine</a> is one of the primary goals of health care reform. Taking care out of clinics and hospitals and letting people care for themselves in their natural environments gives us whole new opportunities for maintaining high-level functioning.</em>&#8221;</p>
<p>I agree. We are always seeing huge leaps in this direction with currently available technologies. For example:</p>
<ul>
<li>I use the <a href="http://myzeo.tellapal.com/a/clk/lt8ZV">Zeo</a> to help me measure my sleep quality and quantify whether my sleepiness on a given day is due to a <a class="zem_slink" title="Sleep deprivation" href="http://en.wikipedia.org/wiki/Sleep_deprivation" rel="wikipedia">lack of sleep</a> or some other issue. This revealed in my case that even after I adjusted for enough sleep I was still experiencing excessive daytime sleepiness (<a class="zem_slink" title="Excessive daytime sleepiness" href="http://en.wikipedia.org/wiki/Excessive_daytime_sleepiness" rel="wikipedia">EDS</a>), which led to doctor appointments, blood tests, and a <a class="zem_slink" title="Antinuclear Antibodies Ana" href="http://arthritis.webmd.com/antinuclear-antibodies-ana" rel="webmd">positive ANA</a> result, and now an impending visit to the rheumatologist to explore whether I&#8217;m suffering from an autoimmune disorder.</li>
<li>I use <a href="http://www.worksmartlabs.com//">Noom</a> to track my exercise and analyze my activity over time. This revealed that I have a REALLY hard time <a class="zem_slink" title="Weight loss" href="http://en.wikipedia.org/wiki/Weight_loss" rel="wikipedia">losing weight</a>. I rode nearly 400 miles in less than 30 days on a <a class="zem_slink" title="Recumbent bicycle" href="http://en.wikipedia.org/wiki/Recumbent_bicycle" rel="wikipedia">recumbent bike</a> and didn&#8217;t lose a single pound.<sup>[<a href="#ehealth-a-bright-future-n-1" class="footnoted" id="to-ehealth-a-bright-future-n-1">1</a>]</sup></li>
<li>I use the web on a regular basis to research health from numerous perspectives to assist myself in maintaining the best health and being an informed patient when I visit the doctor. For example, I&#8217;ve recently been reading about research concerning glutamate in the treatment of depression and ocd.</li>
</ul>
<p>These are just a few small ways in which technology currently available can help us better maintain our health. But I&#8217;m looking forward to what is to come&#8230;.and a lot is in the wings. Consider, just for instance, the growing use of electronic prescriptions which significantly reduce prescribing errors which can have disastrous or even fatal health effects!</p>

<p>I am always shocked when I interact with industries that are lagging far behind the current technological era. Higher Education is one of these areas &#8211; and by this I don&#8217;t mean the educational institutions but rather the software providers. I am repeatedly shocked at the absolutely horrendous software that is pushed onto higher ed. institutions, but that is another topic for another day.</p>
<p>Another area that demonstrates this sort of technological lack is medicine &#8211; especially as it applies to our primary care physicians (PCP) / family doctors. The first glaring sign of this lack is when one attempts to find a doctor. Yes, they show up in the yellow pages and on Google Maps &#8211; but do they have a website? By and large: NO! This is astounding, in my humble opinion, when numerous small businesses, almost every other medium business, and many individuals have websites.</p>
<p>In both cases (higher ed and medicine) I believe this sort of lack is the result of two factors:</p>
<ol>
<li>An ample number of customers mitigating the need for active recruitment.</li>
<li>Key individuals are oftentimes overworked, leaving little time for such &#8220;extraneous&#8221; considerations.</li>
</ol>
<p>In this article I&#8217;d like to make a few suggestions to health care providers on ways they could improve their service. I will do so by chronicling my current personal search for a primary physician in the Langhorne PA area and the various experiences I encounter in this process. I will also attempt to focus on ways to simplify various aspects of the medical practice, from my lay perspective, as I realize that unless something changes key individuals in the medical profession will not have time to read articles such as this much less implement various suggestions.</p>
<p><strong>Get a Website:</strong></p>
<p>A simple website can be had for a few hundred dollars<sup>[<a href="#thoughts-for-primary-care-physicians-part-1-n-1" class="footnoted" id="to-thoughts-for-primary-care-physicians-part-1-n-1">1</a>]</sup> and will greatly increase your visibility to the community and reduce the number of recurring questions one must answer.</p>
<p>I did find that <a href="http://woodlanewellnesscenter.com/">Wood Lane Wellness Center</a> has a website and is in my local area, but this appears to be a more holistic/alternative health venue &#8211; which is not surprising, as there is more pressure for these forms of business to drum up customers than for traditional physicians.</p>
<p><a href="http://www.bucksent.com/">Bucks ENT</a> has a nice website as well, but they are focused on Ear, Nose, and Throat issues rather than primary care &#8211; again, not surprising that they have a website since specialistics, again, must do more work to drum up a customer base.</p>
<p>Now here is a surprise &#8211; and the sort of practice I am looking for &#8211; <a href="http://www.signaturemedicinemd.com/">Signature Medicine</a>. Note the following highlights of this site that demonstrate best practices for other medical practitioners looking to launch a website of their own:</p>
<ul>
<li>The overall aesthetic and design of the site is professional, pleasing, and yet simple.</li>
<li>Information about the practice, including a fairly detailed (while still brief) summary of the doctor&#8217;s experience is presented.</li>
<li>Medical services are listed and areas of special focus are included, thus helping someone quickly determine if the doctor has experience in a specific area.</li>
<li>The online services section indicates that this practice is committed not only to a web-based presence but also to the active utilization of various technologies to facilitate productive medical treatment.</li>
</ul>
<p>Note also these issues to avoid:</p>
<ul>
<li>The press page does not include any articles since 2008. One should &#8220;make news&#8221; at least once a year in some way or other &#8211; even if it is just a small blog post like this one. Otherwise it can appear that the practice is not being currently maintained.</li>
<li>There are several locations where links don&#8217;t work correctly (e.g. the Blog link on the Press page and the Dr. Sitapara link on the About Us page).</li>
</ul>
<p>Some practices are using a third party service such as <a href="http://www.zocdoc.com/">ZocDoc</a> to host their online presence, for example, <a href="http://www.knightsroadmedical.com/">Knights Road Medical Practice</a>. This is okay, but certainly not optimal. Ideally, one should have a presence of one&#8217;s own, as well as presences on various services such as ZocDoc.</p>
<p>Then there is <a href="http://www.newtownmedicalgroup.com/">Newtown Medical Group</a>, they have a website but it doesn&#8217;t say good things about their technological adeptness. This style of website design is 2000&#8242;ish and the site should be completely redone. Note that the navigation changes based on what page one is on, overall the site can be confusing to navigate.</p>
<p>Still, they can&#8217;t be that far behind &#8211; as they do have an online patient portal and allow for virtual visits &#8211; two great services&#8230;but it is unlikely that many folks who are interested in these sorts of services will remain on the site long enough to make this discovery.</p>
<p>One also sees CVS and Walmart entering the fray with options like <a href="http://minuteclinic.com/">The Minute Clinic</a>. While these practices seem efficient and inexpensive, I question their proactivity in providing ongoing care&#8230;thus while I might use one in a pinch, I can&#8217;t see myself relying on one all the time.</p>
<p>Along similar lines to Newtown Medical Group mentioned above is <a href="http://www.woodbournefamily.com">Woodbourne Family Practice</a>. The site&#8217;s design isn&#8217;t as much out-of-date as Newtown&#8217;s, rather it just lacks a few key essentials &#8211; for example, the layout is somewhat confusing with navigation moving around, the color scheme doesn&#8217;t always mesh, and so on.</p>
<p><strong>Conclusion:</strong></p>
<p>I&#8217;ll have to write some additional articles on other areas of technological innovation for PCP&#8217;s, as this post has become quite long enough. Let me just close with these thoughts:</p>
<ol>
<li>PCP&#8217;s are not encouraged to technologically innovate b/c they already have a stable base of clients.</li>
<li>PCP&#8217;s are discouraged from technological innovation b/c they are oftentimes overworked.</li>
<li>The use of technological innovation can increase effectiveness and reduce costs in the long-haul and is a worthy investment.</li>
<li>The first step to a technologically adept medical practice is a web presence.</li>
<li>Your web presence should be fairly inexpensive. For the basics, you&#8217;ll be looking at &lt;$100/yr. for web hosting and perhaps $500-$2,000 for the initial web site design and content &#8211; depending on how much content you desire and what special tweaks one desires.</li>

			<content:encoded><![CDATA[<p>I&#8217;m not a health nut. My mom and one of my sisters are pretty big into health &#8211; <a class="zem_slink" title="Nutrition" rel="wikipedia" href="http://en.wikipedia.org/wiki/Nutrition">nutritional</a>, weight, etc. &#8211; but I&#8217;ve always been too busy to spend the time to plan out meals, think about nutritional value, or even cook. For the last eight months or so my life has been moving at breakneck speed as I&#8217;ve devoted my energies to three major projects and I realized that if I wanted to squeeze more time and productivity out of my day the best way to start was to focus on my health. That is when I discovered <a href="http://www.ichange.com/">iChange</a>. This is only my third or fourth day using iChange, but I&#8217;m already sold.</p>
<p>iChange, like so many other sites, offers guidance on losing weight and eating well&#8230;but it goes so far beyond what other sites are currently offering with a number of unique features. Let me just highlight a few:</p>
<ul>
<li>When you register with iChange you can choose a personal <a class="zem_slink" title="Nutritionist" rel="wikipedia" href="http://en.wikipedia.org/wiki/Nutritionist">nutritionist</a> to assist you. Right now this is either Diana Young (<a class="zem_slink" title="Dietitian" rel="wikipedia" href="http://en.wikipedia.org/wiki/Dietitian">registered dietitian</a>, specializes in ADA methods, served as Chief Clinical Dietitian at two major hospitals) and Linda Miner (registered nutritional counselor, specializes in metabolic typing, holistic and <a class="zem_slink" title="Alternative medicine" rel="wikipedia" href="http://en.wikipedia.org/wiki/Alternative_medicine">alternative medicine</a> approach). These folks are really available to you &#8211; ready to answer questions and give you counsel on how to progress towards better health.</li>
<li>iChange takes a really holistic approach. Many sites focus on <a class="zem_slink" title="Weight loss" rel="wikipedia" href="http://en.wikipedia.org/wiki/Weight_loss">weight loss</a>, mood monitoring, nutritional diets, and so on &#8211; but very few talk a full-fledged and well-rounded approach. iChange offers nutritional/meals modeling, weight loss / exercise, fluids, calorie, mood, <a class="zem_slink" title="Energy" rel="wikipedia" href="http://en.wikipedia.org/wiki/Energy">energy</a>, sleep and stress monitoring.</li>
<li>They offer a simple checklist that only has three items on it at a time that encourage you to take small steps towards increasing your health. As you finish a step you are given another task. You can bang these out or take your time.</li>
<li>There is an incentive system &#8211; its minor (e.g. web-based &#8220;awards&#8221;) &#8211; but still pretty cool. We all like &#8220;earning&#8221; awards or achieving accomplishments (why else do so many people play farmville)?</li>
<li>The community is proactive in reaching out. Within hours of registering I received a contact from another member (a SuperCharger) who asked how I was, introduced herself, let me know how she&#8217;d successfully used iChange, and generally provided encouragement to continue on. This is unlike other communities for health I&#8217;ve been involved in, where there isn&#8217;t that sort of proactive effort to reach out to new members.</li>
</ul>
<p>You might think a site like this would cost something &#8211; many other health sites with more limited functionality do &#8211; or have a freemium model. But iChange is entirely free.</p>
<p>So, my advice &#8211; go register. Give it a try. It takes ten minutes to get yourself through the initial setup and its very intuitive. I think you&#8217;ll find it well worth your time.</p>
<p>In closing, I&#8217;d like to make a list of some enhancements I&#8217;d like to see in iChange. While these are enhancements that should be implemented &#8211; I&#8217;ll reiterate again that iChange is far ahead of any competitors I&#8217;ve seen thus far.</p>
<ul>
<li>The ability to monitor medication intake.</li>
<li>Eventually, the site will need a profit model &#8211; they have a few links to Amazon associate products tucked away, but this really should be tightly integrated into the system.</li>
<li>There is a nice row of boxes along the top with checkboxes that show you your progress in various areas, unfortunately they aren&#8217;t clickable!</li>
<li>The iChange site is a bit slow sometimes. Need to ramp up the server power. <img src='http://www.daveenjoys.com/wp-includes/images/smilies/icon_smile.gif' alt=':)' class='wp-smiley' /> </li>
<li>Links in emails to attached documents take you to the main iChange page &#8211; not very helpful. <img src='http://www.daveenjoys.com/wp-includes/images/smilies/icon_sad.gif' alt=':(' class='wp-smiley' /> </li>
<li>When you download a document, its name is just a string of random letters and numbers, it&#8217;d be nice if the <a class="zem_slink" title="Portable Document Format" rel="wikipedia" href="http://en.wikipedia.org/wiki/Portable_Document_Format">PDFs</a> had names reflecting their purpose. Of course, I can rename them, but still&#8230;</li>
<li>Create a Facebook app. &#8211; this will be a huge marketing item. It will provide individuals who view an iChange profile with a quick and convincing advertisement to use the program &#8211; e.g. when they see we&#8217;ve lost ten lbs using the service and so on.</li>
<li>Eventually, it&#8217;d be pretty cool to see it allow us to &#8220;schedule&#8221; things we should be doing on a semi-regular basis &#8211; e.g. do we need to make an appointment with the dentist, <a class="zem_slink" title="Physician" rel="wikipedia" href="http://en.wikipedia.org/wiki/Physician">doctor</a>, or optomologist? I&#8217;d like to see this expanded to include more general task management &#8211; I used to use a service (whose name I can&#8217;t remember and has since closed) that reminded me of things I needed to do around the house and with my automobile &#8211; allowing me to take better care of my house and belonging &#8211; things like changing furnace filters, checking batteries in smoke alarms, and taking the car in for inspection/maintenance.</li>
<li>I&#8217;ll add more as I think of them.</li>

<p>they can be a part of the solution[1. I do believe in the sickness of mankind - corporately and individually. In other words, I do not believe we are a blank slate capable of perfect good, but rather irretrievably corrupted, redeemable only via super-natural means. In such, I agree with traditional ideas on the depravity on man. However, too often the fact that we are sick has been used to describe us as dead. We yet bear a bit of that perfect image in which we were formed and while we will always make bad choices this does not preclude us from making good choices. Evangelacism too often has failed to lead causes in arenas such as <a class="zem_slink" title="Social justice" href="http://en.wikipedia.org/wiki/Social_justice" rel="wikipedia">social justice</a> because of a misuse of the concept of depravity. I consider myself solidly evangelical but unwilling to allow depravity to overcome ideas of social justice, charity, and humanity. It is a partial explanation of, but not a full picture, the nature and future of man.]. I believe this is the case with the <a class="zem_slink" title="World Community Grid" href="http://www.worldcommunitygrid.org/" rel="homepage">World Community Grid</a> and I want to share with each of you about this exciting project that meets real needs and that each of us can painlessly participate in.</p>
<p><strong>The Problem:</strong></p>
<p>Humans cannot perform the calculations fast enough for many of the massive scientific and medical experiment currently in process. We need the assistance of computers to perform these massive calculations faster and reliably. Unfortunately, <a class="zem_slink" title="Supercomputer" href="http://en.wikipedia.org/wiki/Supercomputer" rel="wikipedia">super-computers</a> with massive <a class="zem_slink" title="Instructions per second" href="http://en.wikipedia.org/wiki/Instructions_per_second" rel="wikipedia">processing power</a> are in limited supply and expensive to either build or rent. This places a strain on medical and scientific researchers as they must secure extra funds or wait for computing resources to become available.</p>
<p><strong>The Solution:</strong></p>
<p>The World Community Grid allows individuals to install an application on their computer that essentially allows their computer to become part of a massive super-computer. Already hundreds of thousands of individuals participate, and well over a million machines are involved. Already we have contributed 195,000 years of processing power to various projects.</p>
<p><strong>The Questions:</strong></p>
<ul>
<li>Will my computer run slower? Yes and no. In a strictly literal sense, it will &#8211; because its doing something else in the background. But in a realistic sense, there are generally no tangible changes in system performance. The software automatically reduces its resource utilization when you are actively utilizing the computer[2. You can always turn off the application temporarily or permanently.].</li>
<li>What sort of projects are these researchers working on? Cures/treatments for cancers, <a class="zem_slink" title="AIDS" href="http://en.wikipedia.org/wiki/AIDS" rel="wikipedia">AIDs</a>, muscular dystrophy, STDs, malaria, and <a class="zem_slink" title="Flavivirus" href="http://en.wikipedia.org/wiki/Flavivirus" rel="wikipedia">flavivirus infections</a> (e.g. <a class="zem_slink" title="West Nile virus" href="http://en.wikipedia.org/wiki/West_Nile_virus" rel="wikipedia">West Nile encephalitis</a> and yellow fever). The projects also model climate predictions and look for effective food hybrids (e.g. a more nutritious rice).</li>
</ul>
<p><strong>Conclusion:</strong></p>

In the U.S. alone we have a population of well over 200 million. Right now there are less than one million participants and we are already accomplishing amazing things.
